Instructions

Fill in: Dog Breed (information on page 2), Colors (up to two colors or may list as tricolor), other ID or markings (if any) and Year of Birth of Dog.

Complete owner Information: Name, Address, Phone

Note type of License with Fees:

If your dog is spayed or neutered, the fee is: $13.50 ($6.00 for Seniors 65+)

If your dog is NOT spayed or neutered, the fee is: $20.50 ($10.50 for Seniors 65+). Service Dogs: New York State requires all dogs over 4 months to have a license, this includes services dogs. There is no fee for service dogs however yearly renewal & proof of rabies is mandatory.

Return license form and payment along with proof from the Veterinarian showing that:

  • Dog is spayed or neutered
  • Rabies vaccine is up to date – proof needs to show date of vaccine

All proof will be returned to you along with the dog license tag. Note: The dog tag/license number is permanent. The only time a dog license tag is replaced, for a fee of $3.00, is if the original is lost.

According to New York State Law all dogs must be licensed.
